Drug Use Notification Form

All Junior Fair participants who need to fill out a DUNF form must fill out the online version - there will be no paper forms accepted. 

Please review these important tips before filling out the online form:

• Please fill out the information for the Junior Fair participant only - parents only need to sign on the last screen.
• The last screen - once you sign and click next then your form will be submitted, there is no opportunity to review
• After you finish the online form, you will receive an email confirmation. Please make sure you complete all the steps.
• For Poultry, any information you don't have do your best or type "NA". The species dropdown, you will choose Market Ducks, Market Turkeys, Market Chickens. Breed, sex and color can be answered to the best of your knowledge or type "NA"
• Once you fill out the online form and sign it, if there is a mistake then you must fill out the form again.
